How German Health Insurance Actually Works
Five things you need to understand before choosing your insurance
It's Mandatory
Everyone living in Germany must have health insurance — Versicherungspflicht. Your employer enrolls you automatically. Self-employed? You choose within 3 months.
Solidarity Principle
GKV follows the Solidaritätsprinzip: the healthy subsidize the sick, the rich subsidize the poor. Premiums are income-based, not risk-based.
Employer Splits the Cost
Employees pay ~half, employers pay ~half. The total rate is 14.6% + Zusatzbeitrag (avg 1.7%). Self-employed pay the full amount themselves.
Free Choice of Doctor
With GKV, you can see any doctor who accepts public patients (most do). No referrals needed for most specialists. Freedom of choice is guaranteed by law.
Comprehensive Coverage
Hospital stays, outpatient care, prescriptions, mental health, dental basics, maternity, preventive checkups — all covered. Some extras require co-pays.
